Responsibilities
The Registered Nurse, RN, Case Manager is responsible for the overall coordination of care for an assigned case load. Registered Nurse, RN, Case Manager goal is to collaborate with patients, families and health care providers to promote health and manage illness.
Facilitates the use of assessment, care coordination for discharge planning transitions.
Needs knowledge of : Case Management, Discharge Planning, Care Coordination, Pre-certification post acute care services.
2 years of Acute Care Case Management experience preferred.

Associate's Degree Nursing required
