  • Schedule : Day Shift 8a-4p
  • NO WEEKENDS
  • MUST HAVE strong experience with substance abuse and mental health patients

Position Summary :

The Licensed Clinical Social Worker collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those patients / clients requiring social work intervention.

The Licensed Clinical Social Worker provides counseling and crisis intervention to patients / clients who have social or emotional problems in a variety of settings across the age span.

Essential Duties and Responsibilities :

  • Provides necessary social work intervention
  • Counsels individuals and families in crisis and help with issues such as poverty, loss and illness
  • Assesses patient's psychosocial needs, plans for, evaluates and modifies interventions to meet the established patient / client goals
  • Collaborates with all disciplines to plan and evaluate team goals for each patient / client
  • Facilitates patient / client and family referrals to community agencies when appropriate
  • Provides financial counseling when necessary to achieve the treatment goals established by the multi- disciplinary team
  • May be required to respond to crisis situations, including domestic violence and child abuse
  • May be required to provide witness services and evidence in court cases
  • Visits schools to assess academic concerns
  • Works with government agencies to help clients apply for Medicare and Medicaid when appropriate
  • Arranges home visits to evaluate a patient / client health, home environment and family interaction / support
  • Assists patient / family / caregivers in planning for present and future psychosocial needs
  • Observes and reports patient progress and / or changes in patient status to physician and appropriate multi- disciplinary team members
  • Adheres to facility and Amergis policies and procedures at all times

Minimum Requirements :

  • Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education required
  • Active license as an LCSW in state of practice required
  • Minimum of one year of social work experience preferred
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit / provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills;

proficiency in the English language is required

  • Computer proficiency required
  • Must be at least 18 years of age
