Schedule Details : Sat and Sunday days 9-1730. On site role at either WMH or PHHC Home Riverwood officeMust be able to train at minimum 2-3 days during the week.
Starting at $19.87 (increases with experience)

Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
Job Summary Coordinates the intake of new patients to Home Health. Assists in future patients planning for services. Responds to community requests for information on resources. What You Will Do :
Coordinates the intake of new patients to Home Health. Seeks out needed information from appropriate resources to complete the intake process.
Collaborates with referral source, physician and patient / family to plan for future agency services.
What you will need :
- High School Diploma or equivalent required.
- Home Health / Hospice experience preferred.
- Customer service experience interacting with patients strongly preferred.
- Referral experience preferred. *CA #LI-SS
