Job Description

Job Title : RN Case Manager – Hospice Trailblazer Wanted!

Pay : $90,000 – $130,000 DOE

Benefits : Full Benefits Package

Location : Serving the Greater Twin Cities Metro Area (21 County Service Area)

Key Responsibilities :

  • Provide expert clinical care to patients with life-limiting illnesses
  • Act as the point person for patients and families navigating hospice
  • Manage medication and symptom relief, ensuring comfort and dignity
  • Collaborate with social workers, chaplains, aides, and physicians
  • Educate caregivers and guide them through emotional and logistical challenges
  • Maintain up-to-date, accurate documentation

    • Qualifications :

  • Active Minnesota RN license
  • 2+ years of hospice, oncology, or palliative care experience
  • Passionate about patient-centered, compassionate end-of-life care
  • Strong interpersonal, critical thinking, and decision-making abilities
  • Commitment to ethical, regulatory-compliant practice
