Qualifications :
- Licensed as a Social Worker through the Board of Georgia
- Possess effective communication skills (including strong verbal and written)
- Possesses a history of building and influencing patient and family relationships
- Holds a current drivers license, car insurance, CPR certification, and positive driving record.
- Hospital, home health, or hospice experience extremely preferred.
Responsibilities :
Assess patient / family social and emotional factors to estimate their potential to cope with terminal
illness and death.
Provide psychosocial counseling and support to the patient / family experiencing emotional, social,
and economic conflict.
Participate in interdisciplinary team (IDT) meeting to establish, review, and revise the plan of care
for each patient, including exchange of information, review of problems, and sharing of
professional support.
- Maintain the dignity of the dying patient.
- Support the patients and family's unique spiritual and cultural beliefs.
- Provide care consistent with National Hospice and Palliative Care Organization standards of
practice for hospice programs.
- Adhere to all Tapestry Hospice policies and procedures.
- Maintain confidentiality of all information pertaining to patients, families, and staff and adhere to all HIPPA rules and regulations
