Position :
NursePractitioner Primary Care Provider
Responsibilities :
- Perform inperson comprehensive assessments includinginitial semiannual and unscheduled assessments and continuousmedical coverage to participants.
- Collaboratewith physicians and IDT to develop care plans for eachparticipant.
- Collaborate with physicians andIDT in discharge planning from acute or longtermsettings.
- Help maintain care of participantsin the nursing home making regular visits as dictated byparticipant need.
- Work collaboratively withWelbeHealth physicians communitybased PACE physicians and IDT tooversee and ensure effective care coordination.
- At the direction of the Medical Director review outcomesand utilization data including hospitalizations and use ofspecialists to establish best practices and policyimprovements.
- Take part in rotating night andweekend call.
Requirements :
Required Qualifications :
- Graduation from an Accredited School ofNursing.
- Certificate as a Nurse Practitionerwith Prescriptive Authority.
- NP StateLicense.
- Minimum of two years of experienceworking with the senior population.
- Currentknowledge of chronic care / geriatric issues and bestpractices.
