Case Manager Home Base ACT Team $2500 sign on bonus

Job Summary :

The Providence Center Case Manager Home Base Assertive Community Treatment (ACT) Team will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric symptoms.

Duties and Responsibilities : include the following. Other duties may be assigned.

Coordinate a variety of services.

Provide supportive counseling, problem solving, and limit setting.

Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with people who have sever persistent mental illness and / or substance abuse issues, as it relates to case management.

In cooperation with managers and emergency services staff, assist with assessments, crisis interventions and risk management as needed.

Teach consumers adult daily living skills.

Initiate treatment and advocate for consumer within network care.

Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.

Transport consumers in own vehicle or Center van to planned activities and appointments.

Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements within required time frames and abide by Center confidentiality policy.

Maintain cooperative relationships with Center staff, consumers, community agencies and the public.

Attend trainings, case presentations and conferences.

Serve on appropriate Center committees.

Requirements :

Associate's degree (AA) is required. Bachelor’s degree (B.A.) from accredited four-year College or University in Human Services, or related field is preferred.

Experience in Human Services, Social Work, or related field is preferred.

Bilingual Spanish preferred.

Must have valid driver's license and insured auto.
