The Social Worker Community Liaison is responsible for providing direct psychosocial interventions and related counseling to patients and their families, including crisis intervention.
Provides group therapy to outpatient population. Performs psychosocial assessments and interventions utilizing psychosocial / social work methods, based on patient's assessed needs.
This position may provide direct patient consultation services and group therapy to adult and geriatric populations.
The social worker provides community resources and referrals. Provides consultation as needed to physicians and other members of the treatment team regarding the continued needs of the patient.
Serves as the key person to provide the facilitation, implementation and coordination of the patient's discharge plans. Participates in treatment planning.
Qualifications :
1+ years’ clinical social work experience in acute care hospital, preferred.
Masters in Social Work from an accredited program by the CA Board of Behavioral Sciences.
