About The Position
Job Details
Requirements :
- Valid Arizona Physical Therapist license
- Minimum of 1 year of experience in a clinical setting
Schedule :
- 13-week assignment
- Start date : Immediately
- 8a - 5p shifts from Mondays - Fridays
Responsibilities :
- Provide physical therapy services to residents
- Conduct evaluations and develop treatment plans
- Treat patients with high-quality practices and care
- Perform chart reviews and document accurately
Additional Information :
- Nursing facility with a staff mix of PT, COTA, PTA, OT, and SLP
- Patient population includes adults aged 18 and up
- Daily patient load of 10-12
- EMR systems : NetHealth & PCC
