Position Summary : This is a TDY grant funded position. TDY Registered Nurse Case Manager / Clinical Diabetes Educator (certified) is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting.
Education is provided via telephonic communication, video teleconferencing, and in-person education sessions. This at times does require village travel.
TDY Educator will be responsible for ordering labs for patients according to hospital policies. TDY Educator will be responsible for documenting education in electronic health record.
This position will assist in the planning, coordinating and providing of diabetes-related health promotion activities in the YK Delta Region.
Patients and activities may involve multiple village trips per month. TDY Educator will work closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care.
TDY Educator assists Diabetes Coordinator with maintenance of the Diabetes Registry and grant requirements. TDY Educator works closely with the Registered Dietitian Clinical Diabetes Educator and Diabetes Pharmacist to ensure comprehensive care of clients.
TDY Educator participates in all program outreach activities such as monthly Diabetes Support Groups and radio shows, media campaigns, and special events.
TDY Educator participates in department planning and goal setting to achieve excellence in patient care and partnership with our tribal leaders.
TDY Educator believes in and promotes Napartet and all corporate endeavors.

Position Qualifications :
Minimum Education : Must have completed a program of education in nursing from an NLN approved institution. BSN required, MSN preferred.
Minimum Experience : Two (2) years experience in a clinical health setting. Demonstrated experience collaborating with a variety of disciplines for project completion.
Background in Diabetes Education required.
License, Certification, Registration : Valid Certified Diabetes Care and Education Specialist credential in addition to : Valid license in good standing in the State of Alaska as a Registered Professional Nurse.
Preferred Basic Life Support (BLS) certification.
Equipment / Tools : Work in a typical office, hospital, and village clinic setting. Comprehensive knowledge of utilization and electronic health records.
Ability to operate diabetes care medical equipment such as glucometer, ht. / weight scale, etc. Requires travel to villages via small plane / sub-arctic conditions, moderate lifting of equipment, occasional long working hours.
Specialized Knowledge and Skills : Strong knowledge base in the field of diabetes and public health. Cross cultural communication skills essential.
Bi-lingual (Yupik / English) preferred.

Agency nurses will need to be prepared to take a math test on medication calculations for adults and pediatrics upon arrival and before working the floor.

a.Right of return transportation. YKHC will provide return transportation (at the cost to the organization) upon termination of an employment agreement unless the termination is a result of :
- i. falsification of employment application;
- ii. violation of the organizations Drug and Alcohol-Free Workplace Policy;
iii. fighting; or
iv. unexcused absence from duties for more than three consecutive scheduled work days
v. voluntary removal from the agency and / or traveler
