QUALIFIED MENTAL HEALTH PROFESSIONAL (QMHP) - Full-Time Greenwood Care, Evanston, IL
Greenwood Care, a Specialized Mental Health Rehabilitation Facility (SMHRF) in Evanston, IL, has an Immediate Opening for a Qualified Mental Health Professional (QMHP).
This is a Full-Time position with a 8a - 4 : 30p Schedule. Hours May Vary.
Up to $ 29.00 per hour !(Depending upon which benefit plan selected )

Responsibilities of the Qualified Mental Health Professional (QMHP) :
- Conduct intake interviews
- Collaborate with clinical team in development and implementation of a service / treatment plan
- May lead consumers in recreational, educational, and / or treatment activities
- Maintain all consumer documentation
- Manage a caseload of up to 30 cases
- Attend clinical team meetings as required

Requirements :
of the Qualified Mental Health Professional (QMHP) :
- Master’s level degree in human services related major
- Employment experience in the field of mental health
- Experience in a Long-Term Care facility
