Summary :
Responsible for providing care management services and support to improve health outcomes via a coordinated approach. The Care Manager works in collaboration and continuous partnership with patients and their family members, as well as primary care providers, specialists, clinic(s), hospital, and post-acute partners, along with community resources, to achieve the desired outcomes.
Using a defined process to identify patients / members at risk for poor outcomes, the Care Manager establishes care plans and goals, and coordinates care and services throughout the continuum of care with the goal of maximizing member health and well-being, improving adherence to health programs, and reducing health care costs.
The Care Manager must be highly collaborative with strong customer service skills and be able to demonstrate the ability to actively engage patients and providers in positive relationships.
Must also be able to demonstrate the knowledge and skills necessary to provide care management services appropriate to the member being served.
Minimum Qualifications :
1. Formal Education Required :
a. Graduate of an accredited school of professional nursing or a school accepted for candidacy status from an accrediting body, bachelor's degree preferred
b. Current license to practice registered nursing in the State of Ohio
2. Experience & Training Required :
a. Two (2) years care coordination to include one year of bedside nursing experience required; case management, chronic disease management, population health, or managed care experience preferred
b. Experience working in interdisciplinary teams
c. Ability to travel from location to location throughout the workday.
d. Remote deployment will be considered
3. Other Skills, Competencies and Qualifications :
a. Computer proficiency required

d. Population Specific Competency : Ability to effectively interact with populations of patients / customers with an understanding of their needs for self-respect and dignity
e. Demonstrates knowledge of healthcare managed care, the care continuum and the transition of patients along the various levels of care
f. Demonstrates knowledge of care management strategies and the management of chronic conditions
g. Possesses excellent organizational skills
h. Possesses excellent collaboration skills
i. Demonstrates superb relationship building skills and will frequently be required to calm and gain confidence of others
j. Demonstrates the ability to work in a self-directed manner and must demonstrate independent judgment and critical thinking in decision making and problem solving related to implementing plans, programs, project participation, and use of technical systems with and across the New Health Collaborative
k. Demonstrates knowledge of medical necessity in relationship to healthcare resource utilization
l. Ability to communicate instructional and operational information verbally and in written form that may include correspondence, reports, instructional materials, system designs
m. Requires excellent attention to detail
n. Flexible : ability to adjust work hours to meet business demands
o. Ability to maintain confidentiality of health and business information
4. Level of Physical Demands :
a. Sedentary : Exerts up to ten pounds of force occasionally and / or a negligible amount of force frequently

$30.60 / hr - $45.90 / hr
The salary range on this job posting / advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay.
We believe in the importance of pay equity and consider internal equity of our current team members when determining offers.
Please keep in mind that the range that is listed is the full base salary range. Hiring at the maximum of the range would not be typical.
