DUTIES :
- Provides advice & support to the Residents and theirfamilies.
- Monitoring Residents andtreatments.
- Documents Resident care servicesby charting in Resident & dept. records.
- Protects Residents & staff by adhering toinfectioncontrol policies & protocols.
- Resolves Resident and family problems & needs byutilizing multidisciplinary team strategies.
- Assures quality of care by adhering to Facilityphilosophies & standards of care.
- Maintains Resident confidence by keeping informationconfidential.
REQUIREMENTS :
- Minimum of 1 year experience in Long Term CareSetting.
- Sub Acute Unit Experience in an LTCfacility preferred.
- PCC knowledge highlypreferred.
- Should work well in a teamenvironment.
- Current New York State RNLicense.
- Solid computer skills; workingknowledge of MS Office.
- Excellentcommunication skills.
- Should be friendly anda team player.
