Registered Nurse Care Coordinator/Case Manager
OverviewAnsible Government Solutions (Ansible) subsidiary Titan-Auxo, LLC is currently recruiting a Registered Nurse Care Coordinator / Case Manager to support telehealth services for the Durham VA Healthcare System in Durham, NC.
Working hours are Monday-Friday 8 : 00-4 : 30pm EST. If you accept employment with Ansible, you must also acknowledge that any assigned schedule is subject to change in the direction of either Ansible or its customers.
Responsibilities : Determine patient medical necessity.Conducting care coordination and educating patients on their condition, treatment options and benefits.
Develop, monitor and review treatment plans.Ensure that all incoming consults are triaged within two business days of receipt.
Contact patients with negative results within three business days of when they are returned.Update patient tracker with relevant patient information with minimal errors.
Ensure that consults to any other service are input and read for signature.Requirements : Ability to successfully undergo a Government sponsored background investigation (US Citizenship Required)Associate Degree or Bachelor of Science in Nursing from an accredited program, college, or university and hold a current license to practice as a Registered Nurse in any State;
a Bachelor’s degree is preferredExperience with RN activities in providing hematology / oncology care for a minimum of 2 yearsTelehealth Care Coordination / Case Manager experience requiredSpecialization in GeneticsBasic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) CertifiedFamiliarity with medical terminology, hospital terminology and / or clinics.
Technically proficient in the skills necessary to fulfill the government's requirements; will be provided training by the VA on appointment management, VA policy and procedures.
