Certified Occupational Therapist Assistant (COTA) in Skilled Nursing Facility
Now Staffing Certified Occupational Therapist Assistant (COTA) for Rehab Departments throughout NYC and Long Island.
Flagstar Rehab COTAs work with a diverse and multi-disciplinary team to perform treatments and patient / family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.

Hourly Pay Rate : $33 (W-2), $37 (1099)
Occupational Therapist Assistant Job Duties :
- Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
- Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
- Monitor and document patient progress with the use of innovative EMR software.
Occupational Therapist Assistants employed by Flagstar Rehab will receive :

Occupational TherapistAssistant Requirements :
- Current NYS OTA License and Registration
- EMR Competence
- State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
- Valid Proof of Identity and Work Authorization
- Malpractice / Liability Insurance
