We are looking for a Palliative Care Nurse to provide high-quality end of life care to patients and their families. You will conduct assessments, provide education, and coordinate care to ensure patients are comfortable in their home environment.

Your Responsibilities :
- Conduct in-person and telephonic assessments for palliative care patients
- Provide patient education and support
- Coordinate care with the interdisciplinary team
- Document timely clinical records
- Facilitate care plan development and implementation
Your Qualifications :
- Registered Nurse with 2 years of prior experience
- Experience in palliative care, oncology, hospice, or geriatric care preferred
- Strong compassionate communication skills
- Competent in computer programs and electronic medical record documentation
- Current RN license and CPR certification
