Registered Nurse Case Manager
Join our team as a Registered Nurse Case Manager and make a difference in the lives of home health patients. Responsibilities include providing skilled nursing visits, administering ongoing care, coordinating healthcare personnel, and documenting patient care.
A minimum of one year of nursing experience is preferred, along with strong organizational and communication skills. A valid driver's license and reliable transportation are also required.
Required Experience / Skills :
- Diploma, Associate, or Bachelor Degree in Nursing
- Minimum of one year nursing experience preferred
- Strong med surg, ICU, ER, acute experience
- Home health experience is a plus
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
Scheduled Weekly Hours : 40
Pay Range : $69,800 - $96,200 per year
