Description
Great opportunity to join an experienced team supporting transitioning planning for medical / surgical inpatients.
Role requires ability to work independently and ability to prioritize tasks.
Work closely with members of the care team - interact with patients -good communication skills and ability to use computer are key
Supports the Case Manager in coordinating, negotiating, procuring and managing resources to support the care of complex patients, in facilitating achievement of quality care and patient cost outcomes.
Works collaboratively with interdisciplinary staff internal and external to the organization to ensure efficient utilization of health care resources including, but not limited to, acute, intermediate and chronic care services, specialty programs and services, utilization of healthcare personnel and material resources.
Participates in quality improvement and evaluation processes related to the management of patient care.
Provides essential support to the Mission of Salem Hospital and goals for high quality care that is efficient, effective and results in a high rate of customer satisfaction.
Supports compliance with applicable criteria, guidelines and regulatory requirements for patient management. Works with Case Manager to manage available resources to facilitate optimal outcomes for the well being of targeted patient groups.
Assists Case Manager in implementing a comprehensive and effective framework for case management designed to promote and support wellness among service users and maximize reimbursement for the Medical Center.

Qualifications
To perform this job successfully, be able to demonstrate knowledge and skills necessary to assist the CM coordinate resource utilization within the continuum of care that is appropriate to the age and needs of the patients served by the program.
Mature professional with high level of personal energy and professional motivation. Knowledge of payer requirements, vendor services and the systems and processes related to obtaining those services.
A commitment to cost-effective patient care and patient education is essential, as is the ability to work independently within a team-oriented, structured environment.
Excellent interpersonal and communication skills. Computer literate, ability to quickly gain competency in various hospital software applications.
Ability to operate telephone equipment, fax machine, copy machine and calculator. Requires the ability to simultaneously track and coordinate multiple details.
Ability to discern important details and facts from fast-paced conversation and available documentation. Ability to consult with others.
Good critical thinking skills; creative, organized approach to all work tasks.
One or more years experience in a service / healthcare industry environment is required. High school diploma or equivalent is required.
