Position Summary : The Registered Nurse (RN) Public Health Case Manager oversees comprehensive case management for individuals requiring public health care such as well-child checkups, immunizations, tuberculosis screening, and management of chronic health conditions.
The role entails training Community Health Aides and Practitioners (CHA / Ps) to competently administer public health testing, examinations, and immunizations.
Responsibilities of the position includes participating in the design, execution, monitoring, quality assurance, and assessment of the Well Child / Immunization and other public health programs.
This includes identifying areas of improvement and proposing effective strategies to enhance program performance.
The incumbent is expected to travel to various villages to render services and provide on-site instruction, and supports CHA / Ps in conducting examinations, assisting with testing, and administering immunizations, ensuring adherence to health and safety protocols.
The Case Manager provides education on topics related to immunizations, chronic care, preventive health examinations, and diagnostic and surveillance testing, and is required to communicate effectively with diverse audiences, ensuring the information presented is understood and applied appropriately.
Additionally, the Case Manager assists in providing instruction during Health Aide Training and at Continuing Medical Education (CME) events for CHA / Ps and other clinical staff.
Other duties include coordinating with other health care providers, maintaining accurate records of patient interactions, monitoring patient progress, and ensuring patients' health needs are met effectively.
The Case Manager must stay updated with the latest public health knowledge and developments, applying this knowledge to improve patient outcomes and overall program effectiveness.
Position Qualifications :
Minimum Education : Diploma or AAS for RN, prefer BSN
Minimum Experience : 2 years of positive work history as RN.
License, Certification, Registration : Must have current licensure in State of Alaska. Current BLS, recommend ACLS and PALS
Equipment / Tools : Must have computer experience. Proficiency in MS Office software including WORD, EXCEL and ACCESS preferred.
Must be adept with RPMS and its modules within 3 months of hire. Must be proficient with equipment and tools used in an EPSDT exam within 3 months of hire.
Specialized Knowledge and Skills : Knowledge and experience with Pediatrics required. Prefer experience in teaching, working in cross-cultural setting and knowledge of the Community Health Aide Program.
Excellent verbal and written communication skills. Fluent in English.
