Job Description :

We are seeking experienced and compassionate Home Health Nurses to join our team. As a Home Health Nurse, you will be responsible for providing skilled nursing care to patients in their homes, under the direction of a physician.

You will assess patients’ health needs, develop and implement care plans, and collaborate with other healthcare professionals to ensure optimal patient outcomes.

Responsibilities :

  • Conduct initial and ongoing assessments of patients’ physical, emotional, and psychosocial needs
  • Develop individualized care plans based on assessment findings and in consultation with physicians and other members of the healthcare team
  • Provide skilled nursing care, including wound care, medication management, and monitoring of vital signs
  • Educate patients and their families about their health conditions, medications, and self-care techniques
  • Coordinate with other healthcare professionals, such as physical therapists, occupational therapists, and social workers, to ensure comprehensive care
  • Document patient assessments, interventions, and outcomes accurately and timely in accordance with agency policies and procedures
  • Communicate effectively with patients, families, and other members of the healthcare team
  • Participate in quality improvement activities and contribute to the development of best practices in home health care

Qualifications :

  • Current license to practice as a Registered Nurse (RN) in the state of Oregon
  • Minimum of one year of experience as a nurse in a clinical setting; home health experience preferred
  • BLS certification required; additional certifications such as ACLS and wound care certification are a plus
  • Strong assessment, critical thinking, and communication skills
  • Ability to work independently and as part of a multidisciplinary team
  • Compassionate, patient-centered approach to care
  • Reliable transportation and valid driver’s license
