Physical Therapist (Full-Time)
Location : Middletown, OH Serving 100% Senior Living Communities
Position Overview :
We are seeking a dedicated Physical Therapist to provide outpatient therapy in a home-based setting within the Middletown, OH area.
This is a full-time role requiring a minimum of 30 hours of direct patient care per week , with services billed under Medicare Part B .
This is an exciting opportunity to make a difference in the lives of older adults by delivering therapy in Senior Living Communities (SLCs) .

Minimum Requirements :
- Doctor of Physical Therapy degree (or equivalent)
- Ohio state licensure eligibility as a Physical Therapist
- Availability for at least 30 hours / week of direct patient care
- Strong commitment to professionalism , accuracy , and dependability
- A genuine passion for evidence-based care
