Job Summary
The Home Care Occupational Therapist provides therapy to patients based on patient need and medical direction. They also provide patient / caregiver education and arrange for adaptive equipment as needed.
Essential Functions
- Perform evaluation procedures to assess the level of disability, create treatment plans, and make appropriate recommendations.
- Provide treatment programs and education to restore maximal function to the patient, assess treatment effectiveness, and modify goals / treatment methods as needed.
- Document evaluation and treatment of each patient in compliance with department standards.
- Instruct and train other rehabilitation staff, medical personnel, and students regarding patient needs and rehabilitation services.
- Participate in process improvement, patient satisfaction initiatives, program development, and marketing activities.
- Demonstrate productivity as indicated by department standards.
- Improve skill areas through continuing education, competencies, and developing new concepts of patient care.
Qualifications
- Bachelor's Degree or equivalent from an accredited occupational therapy program (Required)
- Master's Degree (Preferred)
- 1 year or more of relevant experience within Home Care / VNA (Preferred)
- Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy (Required)
- Basic Life Support (BLS) - AHA American Heart Association or Basic Life Support (BLS) - ARC American Red Cross within 90 days of Hire (Required)
- Driver's License - STATE MI State of Michigan Home Care Business Lines Upon Hire (Required)
