Physical Therapist Assistant – SNF

Location : Fork, SC

Schedule : Full-time

Setting : Skilled Nursing Facility (SNF)

Overview :

We’re seeking a dedicated Physical Therapist Assistant (PTA) to join a skilled nursing facility team in Fork, SC . This role provides the opportunity to assist residents in improving their mobility, function, and quality of life through personalized therapy under the supervision of a Physical Therapist.

Responsibilities :

Assist in delivering physical therapy treatments and interventions in the skilled nursing setting

Implement individualized care plans developed by the supervising Physical Therapist

Collaborate with interdisciplinary team members to support comprehensive patient care

Maintain accurate documentation following state and facility guidelines

Educate patients and caregivers on therapeutic exercises and strategies to promote daily function

Qualifications :

Active South Carolina PTA license

Experience in skilled nursing or geriatric care preferred

Strong communication and teamwork skills

Ability to work independently while following clinical direction
