About The Position
Job Title : RN Case Manager Job Specialty : Case Management - Rehabilitation Job Duration : 13 Weeks Shift : 08 : 00 AM - 04 : 30 PM Guaranteed Hours : Minimum of 32 hours per week Experience : 1-2 years;
inpatient rehabilitation experience highly preferred License : Licensed as RN in the relevant state Certifications : N / A Must-Have : IP Rehab experience preferred Job Description : Manage patient care plans and facilitate communication among team members.
- Coordinate with multidisciplinary teams to ensure seamless patient transition.- Develop discharge plans in collaboration with patients and families.
- Monitor patient progress and adapt care plans as necessary.- Assist in managing patient flow within the rehabilitation unit.
- Maintain precise and up-to-date patient records using the EPIC EMR system.- Adhere to professional and business casual dress code standards.
- Participate in team meetings and contribute to continuous quality improvement initiatives. Additional Information : Position is based in an inpatient rehabilitation unit with a maximum census of 13 beds.
- No floating to other units required.
