Responsibilities

Collaborate with the interprofessional team to delivery high quality person-centered care .

Assist patient with accessing community-based resources by supporting them with applications , transportation needs and follow up on any referrals made to ensure patient utilizes identified resources .

Enhance patient's ability to effectively communicate with healthcare provider

Works with staff and providers to facilitate a care plan and communicates with them regarding the status of efforts

Support patient understanding of educational materials and resources about diabetes

Support patient understanding of educational materials and resources about diabetes

Reinforces standards of care for diabetes including preventative care , annual monitoring and follow up care

Reviews medication plan with patient to confirm understanding and makes appropriate connections with prescribing provider when necessary

Assists patients with recognition of the barriers and implementation of strategies for healthy eating

Qualifications

CMA, LPN , Pharmacy Technician, EMT / Paramedic

Diabetes Community Care Coordinator Certification required within 60 days of hire

Community Health Worker certification within 90 days of hire

Mandatory Reporter certification
