About the Role :
We are seeking a licensed Nurse Practitioner to lead the medical components of our client's team-based care model. This pivotal role involves direct clinical assessments, care coordination, and continuous support of clients’ medical and behavioral health needs.
Key Responsibilities :
- Clinical Assessment : Conduct comprehensive evaluations, combining behavioral and physical assessments to identify unmet health needs and initiate appropriate interventions.
- Team Leadership : Provide guidance and clinical support to Guides and Community Resources Guides, enhancing the integrated care approach.
- Care Coordination : Facilitate seamless collaboration among clients’ healthcare providers to ensure cohesive care plans and effective service delivery.
- Community Engagement : Engage with clients in their environments, whether in the field or at home, to tailor interventions and support directly to their needs.
Ideal Candidate :
- Empathetic and approachable, with excellent communication skills.
- Experienced in working within multidisciplinary teams, comfortable in varied roles including direct service, teaching, and care management.
- Knowledgeable about trauma-informed care, social determinants of health, and data-driven care practices.
- A background in managing underserved populations with behavioral health and / or substance use disorders (SUD) conditions.
- Proficiency with Microsoft Office Suite and ability to operate effectively in non-traditional work conditions.
Qualifications :
- At least 5 years of clinical experience as a Nurse Practitioner, preferably in a primary care or similar setting.
- A valid driver’s license and insurability under our client’s coverage.
- Familiarity with local healthcare providers and community resources.
