CHILDREN CARE MANAGER RESPONSIBILITIES
- Perform all enrollment activities as required by the Department of Health (DOH) and Lead Health Home requirements. This includes outreach contacts, obtaining required signatures on consents, and assisting in obtaining supporting documentation.
- Conduct Child Adolescent Need and Strengths- NY (CANS-NY) and comprehensive assessments as mandated by DOH and Lead Health Home.
- Provide a designated number of Health Home core services via face-to-face interactions with members and other modalities based on acuity scores, as required by DOH and / or Lead Health Home.
- Responsible for creating, developing, and implementing a comprehensive, family-driven, and youthguided Plan of Care (POC).
- Coordinate and conduct Interdisciplinary Team Meetings to facilitate POC creation and service coordination.
- Coordinate care and services among all providers and community resources. Assist with coordination of services and benefits in case of hospitalization or admission to any facility / residence.
Facilitate discharge from any hospital / facility and assist with transfers to another Care Management Agency (CMA) or program if necessary for continuity of care.
- Act as an advocate for the member / parent / caregiver / supports in various institutional settings (i.e., school), within the home or community, while interacting with different providers or attempting to connect the member with additional services or resources.
- Assist the member in obtaining and / or maintaining public benefits, including but not limited to Medicaid, Social Security Benefits, and Office of People with Developmental Disabilities (OPWDD).
- Educate the member / parent / caregiver / supports on the member's diagnosis or condition, as well as their needs, strengths, and goals.
- Make referrals to services and additional resources, assisting the member in following through with services or appointments.
- Meet all monthly billing requirements and documentation as stipulated by DOH and Lead Health Home.
- Maintain accurate case records and document all services.
- Attend regular supervision sessions with a direct supervisor.
- Perform additional duties as assigned.
REQUIREMENTS
- Must be open to working 40 hours per week, as this is a full-time position.
- Must be willing to travel within selective boroughs of NYC (Bronx, Manhattan, Brooklyn, Queens & Staten Island).
- Minimum requirement : Bachelor’s degree or experience working with populations with complex medical, mental health, and psychosocial needs (experience with children and adolescents is a plus).
- Proficiency in modern communication, including but not limited to the usage of a computer, Office 365, and Healthcare Management system.
- Fluency in both English / Spanish or English / Arabic (spoken and written) is a must
Starting pay is $26 / hr
