Summit Home Care is a RN-owned and operated, Joint Commission Accredited, NYS licensed, home care agency that services all 5 Boroughs in NYC and Long Island.
We are seeking Registered Nurses to join our agency who have the drive and desire to help people.
RN / Registered Nurse Job Responsibilities :
- Perform health assessments for patients in their home to assess a patient’s health care status and needs at start of care and then every 6 months and as necessary
- Initiate the plan of care and makes necessary revisions as patient status and needs change
- Document patient history, symptoms, assessments, tests, and care according to guidelines
- Supervise paraprofessional staff on patient plan of care
- Be a Care Manager for your patients
- Educate patients and their families on proper home health care strategies and procedures
- Reporting all issues and concerns to the Director of Patient Services
- Complete documentation accurately and submit timely to agency
- Provide the highest quality care in accordance with DOH regulations
RN / Registered Nurse Requirements :
- Must possess a current, valid NYS RN license and Registration
- Government Issued ID, Social Security Card & Work Authorization
- 2 Reference Letters
- Recent Physical
- TB Screening (Quantiferon required)
- Rubella & Rubeola titer lab work
- Excellent communication skills
- Strong critical thinking skills
- Able to function independently with minimum supervision
- Dedicated to providing the best patient care possible
- Ability to travel to patient’s homes

Pay Range per Visit :
If you are applying to the per diem per visit version of this job, the salary is : $75 per Home Visit minimum 5 visits per week, maximum 5 visits per day
