Registered Nurse- RN- Case Management Full and Part Time Days available Portland, Oregon Relocation and 6k sign on bonus Responsibilities :
- Screen patients for post-acute care needs
- Develop transition plans and coordinate care
- Educate patients and families
- Make referrals to community care services
- Continuously monitor, update, and reassess care plans Required Qualifications :
- Nursing degree / diploma required upon hire.
- Bachelor's degree in Nursing or higher within 3 years of hire.
- Oregon Registered Nurse License upon hire.
- 2 years of Acute care hospital or community-based experience.
- 1 year of experience with electronic medical records or documentation systems.
- 1 year of Home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience.
