Job Duties Include :
- Care for patients in their homes
- Day-to-Day interaction with patients
- Review symptoms and plan of care with patients
- Check patient vital signs
- Protect patient rights and serve as patient advocates
- Educate patients on healthy lifestyle habits
- Complete documentation in patient electronic medical record
- Provide wound care and dressing.
- Create an environment of open communication

Pay : $51,000.00 - $60,000.00 per year
