An experienced and dedicated Home Health Registered Nurse (RN) is sought to provide skilled nursing care to patients within their homes. - Conduct comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.\n
- Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management.\n
- Monitor patient progress, identify any changes in condition, and promptly communicate with physicians and care team members.\n
- Educate patients and families on disease processes, treatment regimens, medications, and preventive care strategies.\n
- Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB).\n
- Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides to ensure holistic care.\n
- Participate in case conferences, staff meetings, and continuing education opportunities.\n
- Adhere to all regulatory guidelines and agency standards related to patient care and documentation.\n\nQualifications and Desired Experience : \n
- Active RN license in good standing within the state of practice.\n
- Minimum one year of recent home health experience; field experience is preferred.\n
- Proficiency with Home Care Home Base (HCHB) software.\n
- Experience with OASIS assessment tools.\n
- Strong clinical assessment abilities with excellent critical thinking.\n
- Valid drivers license, reliable transportation, and auto insurance.\n
- Ability to work independently while managing a flexible field-based schedule.\n
- Excellent communication, organizational, and time management skills.\n
- BSN preferred.\n
- Familiarity with Medicare and home health compliance standards is advantageous.\n
