Position Summary : This is a field-based role for a Certified Nurse Practitioner (CNP or AGNP) interested in wound care and long-term primary care. No certification in wound care? No problem. We’ll help you get there—and cover the cost. Key Responsibilities : Visit multiple senior care sites each week Deliver in-place wound evaluations and follow-up care Participate in care coordination and medication reconciliation Build trusted relationships with residents, families, and facility teams Qualifications : Current MN CNP license Wound care certification or willingness to obtain Background in long-term care or family / geriatric practice Must have reliable transportation Military Occupational Classification (MOC) Codes : Army : 66P Navy : 1972 Air Force : 46Y3H Marines : NP Coast Guard : HS Join Our Team : You became an NP to make a difference.
