If you are located in the Bristol County, MA area, you will have the flexibility to work a Hybrid schedule as this is a Field-based opportunity with a Home-based office as you take on some tough challenges.

THIS IS A FIELD-BASED ROLE WITH A HOME-BASED OFFICE

Primary Responsibilities :

  • Assess, plan, and implement care strategies for members that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
  • Expect to spend about 50-80% of your time in the field visiting our members in their homes or in long-term care facilities

Required Qualifications :

  • Current, unrestricted RN license in MA plus 2+ years of clinical work experience
  • 2+ years of experience in long-term care, home health, hospice, community health, or assisted living
  • 2+ years of experience working with MS Word (create and edit) and Outlook (email and calendar)
  • Ability to travel in assigned region to visit members in their homes and / or other settings, including community centers, hospitals, or providers' offices
  • Access to high-speed internet at residence

Preferred Qualifications :

  • BSN Degree or working towards a BSN
  • Experience with electronic charting
  • Field based work experience
  • Background in managing populations with complex medical or behavioral needs
  • Bilingual
