About The Position

PRN experience in Skilled Nursing Facilities preferredLicense : Must have a valid state licenseCertifications : Covid test required 72 hours prior to start date with results issued 48 hours priorMust-Have : - Resume - License verification - Prescreen documentation Job Description : - Ensure all credentialing requirements are met and attached upon submission.
- Conduct patient assessments to determine suitable physical therapy treatments.- Develop personalized care plans to address patient needs.
- Monitor patient progress and adjust treatments as necessary.- Collaborate with a multidisciplinary team to provide comprehensive patient care.
- Maintain accurate patient records and documentation.- Participate in team meetings and contribute to patient care decisions.
- Ensure compliance with all regulations and standards.- Maintain patient confidentiality and professional conduct.Credentialing Checklist : - State license verification - Resume - Three professional references - County, federal, and multi-state criminal searches (7 years or more)- Nationwide sex offender search (7 years or more)- Health and Human Services / OIG search- General Services Administration search- Social Security Number trace- 1-step PPD or Quantiferon or proof of positive PPD and chest X-ray - SAM search- Minnesota state background check with fingerprintsPosition Urgency : Rapid Response CrisisBilingual : Not requiredHoliday Coverage Required : No
