RN Case Manager (FT-) - Home Health Agency

Home Care Network (HCN) is urgently seeking a highly skilled and experienced Home Health RN Case Manager to join our agency to deliver and lead exceptional care for our existing and new clients across Southeast Ohio, primarily serving Jackson, Vinton, and Gallia Counties.

This field staff leadership role includes starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, as well as new hire orientations, employee reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. The right RNCM has a passion for delivering consistently excellent care and care experiences to clients, and the ability to do so directly and through the thoughtful leadership of others.

More about the role :

• This is a Full-Time, Salaried position.
• Expertise in wound care, case management, IV, lab draws, OASIS, and HHA supervision i s required (1+ years minimum of each).
• Minimum availability required : Monday - Friday from 8 AM - 5 PM.

No On-Call requirements / No Overnight shifts required.

Other Perks :

Compensation : FT RNCM salary starts at $72,800 per year, with quarterly bonus opportunities available.

Qualifications :

Supervisory / DON experience is a plus.
