Copy of certifications required or must be willing to obtain before starting Must-Have : Candidates need both Care Coordination and Utilization Management experience Job Description : Identify, initiate, and manage optimal patient flow / throughput to enhance continuity of care.
- Plan and facilitate coordinated, safe transitions to the next level of care required.- Promote patient satisfaction and quality outcomes.
- Target an optimal length of stay based on the patient's individual response to treatment, procedures, and interventions.
- Maximize contracted benefits and available services in care management planning.- Provide comprehensive care management planning.
- Work schedule includes weekends, specifically Saturday and Sunday, two days per week until the end of 2024.- Scrubs / Apparel should not have any agency or brand logos on them.
This policy is strictly enforced.- Candidates are expected to be able to float within their scope as per job description.
