Key ResponsibilitiesCoordinate care for members by screening for gaps in care and addressing social determinants of healthProvide behavioral health benefit education and network referrals to community resourcesDocument interactions and provide administrative support to the Behavioral Health Case Management departmentRequired QualificationsHigh School Diploma or GED2+ years of managed care or call center experienceProven familiarity with working on computers3+ years of experience in a medical / behavioral health setting preferredDemonstrated casework experience in social service agencies or community mental health centers preferred
