As a Care Manager Registered Nurse, you will be responsible for conducting comprehensive assessments to determine appropriate services and benefits for enrollees based on medical necessity.
Your role involves coordinating and overseeing healthcare services to deliver optimal care while ensuring compliance with state and federal regulations.
You will also be educating and transitioning enrollees to clinical services as per Department of Health guidelines.
Key responsibilities include identifying and coordinating enrollee services, educating enrollees on necessary consents and documentation, monitoring enrollees for changes in condition, developing and revising enrollee-centered Plans of Care, and maintaining documentation using an Electronic Medical Record system.
Requirements include computer literacy, strong communication skills, organizational and time management abilities, and a valid New York State RN license with 1-2 years of experience in a community-based care or healthcare setting.
