Title : Physical Therapist Occupational Health

Location : MT. Vernon, IL

Status : Full Time

Hours : M-F Day shift

Early-career Physical Therapist. Focus in this role is understanding and integrating ATI's Care Model into standard practice, leveraging available tools and resources to guide and optimize care plans and patient outcomes / experience.

Responsibilities :

• Performs evaluations and establishes a goal-based plan of care
• Implements regular objective reassessment points to track progress towards goal achievement and / or adjusts goals based on patients' recovery trajectory
• Establishes care plans aligned to ATI Best Practice guidelines, to achieve predictable and positive outcomes
• Manages his / her Caseload effectively, across the spectrum of each patient's plan of care
• Effectively delegates treatment among the clinical care team
• Meets or exceeds established performance expectations, including patient outcomes collection targets, scheduling practices, productivity, caseloads management, and documentation compliance
• Communicates effectively and regularly with referral sources and other relevant stakeholders in a patient's plan of care (e.

g., physician, family members / caregivers, case manager, attorney, etc)

Assists with daily clinic operational needs and tasks as needed

Qualifications : Minimum Education

Minimum Education

Required :

Minimum Bachelors of Physical Therapy from an accredited institution

Minimum Experience

Required :

No prior experience required as a licensed PT if graduation requirement is met
