To provideclinical nutrition support care and information to patients andstaff of Bay Area Hospital
Performs nutritionassessments & reassessments on assigned units
Evaluates / recommends aggressive nutrition interventionand provides followup for continuity of care
Counsels patients and family members in nutrition principlesmodifications and food selection adapting plans for individuallifestyles and
using appropriate writteninstruction material
Communicates appropriatenutritional care data through patient medical record
Documents using the Nutrition Care Process andstandardized language
Communicate nutritioncare plan to the appropriate physician or support staff
Assists Clinical Nutrition Coordinator inmaintenance / revision of diet manual menus policy and proceduremanual standards of care
educational materialsand Quality Performance / Improvement data collection
Interprets evaluates and uses pertinent current researchrelating to nutrition based on current professional standards andthrough
Continuing Education programs pertinentto areas of practice
Completes nutritiontherapy to achieve desired patient goals and documents evaluationsdaily progress notes treatment plans and
discharge summaries within diet manual guidelines andpolicy & procedure protocols
Participatesin nutrition education orientation and inservice programs for staffpatients students and community
ProvidesMedical Nutrition Therapy and Diabetes SelfManagement training forindividuals & groups
Participates anddelivers longterm care and other nutritional services consultingagreements
Performs other duties asassigned
SUBMISSION REQUIREMENTS
- Bachelors degree in Food & Nutrition or DieteticsREQUIRED
- Dietetic internship or approvedmaster program with qualifying work experience REQUIRED
- 1 year experience in clinical or acute settingREQUIRED
- Must have or be able to obtain anOregon Food Handler Food Manager Certification or ServSafeCertification
