Job Description : Soliant Health is currently seeking a dedicated Physical Therapist to join our team in Westport, CT. As a Physical Therapist, you will work with patients of K-12 ages (preschool openings also available) to restore mobility, reduce pain, and improve overall function.
You will assess patients' needs, develop personalized treatment plans, and provide therapeutic interventions to help them achieve their rehabilitation goals.
Position Details :
- 2024-2025 school year
- Full-time positions
- 3 PT's needed
- 2 at the preschool level
- 1 for the whole district (K-12)
- Onsite only
- PT duties consist mostly of providing therapy
Requirements :
- Master's degree (or higher) in Physical Therapy from an accredited program
- Valid license to practice Physical Therapy in the state of Connecticut
- Previous experience in a school setting preferred
- Familiar with IEPs
Responsibilities :
- Conducting thorough evaluations to assess patients' physical abilities, limitations, and needs
- Developing individualized treatment plans based on assessment findings and patient goals
- Implementing therapeutic exercises, manual therapy techniques, and modalities to address patients' specific needs and promote recovery
- Monitoring and documenting patient progress, adjusting treatment plans as needed to ensure optimal outcomes
- Educating patients and their families on home exercises and strategies to facilitate continued progress and independence
- Collaborating with interdisciplinary healthcare teams to coordinate care and promote comprehensive treatment plans
