Freedom Home Health Care has an immediate need for an experienced Home Health Nurse. This position will lead a team of nurses to manage a group of 35-40 clients.
The Case Manager is responsible for developing the patient care plan using a holistic approach to care in the home. Coordinating care upon admissions, transfers, discharges along with routine visits.
This position will complete home visits to set up medications, wound care, and patient teaching in conjunction with visit nurses to facilitate collaboration in providing quality care.
Freedom Home Health Care offers a base salary for minimum acuity with bonus per diem compensation to reward staff whose experience and ability to navigate the electronic health record cultivates efficiency.
