Job Description : As a Mental Health Counselor / CNA with us, you will know how to : Provide care through effective planning, intervention and evaluationMeet the needs of the patient, family, and multi-disciplinary team through effective communication, coordination, and documentation and under direct supervision of a registered nurseCoordinate patient care and report patient changes to the appropriate team membersEnsure that the progress of patients is communicated to staff, physicians, other departments and familyProvide patient with assigned activities and tasksDocument patients condition in medical and facility records according to policy and regulationsProvide therapeutic interventions on an individual and group basis which are congruent with team structure and program expectationsParticipate in the development and implementation of treatment plans when requestedMaintain under the direction of the nurse, ongoing communication with internal and external individuals and / or agencies as appropriate, utilizing effective customer service skillsEnsure compliance with applicable regulatory guidelines and requirementsDemonstrate age related care skills as defined by unit patient populationsMinimum QualificationsHigh School Diploma or equivalentMinimum of one (1) year Mental Health Counseling inpatient experience or three (3) years memory care / skilled nursing facility with behavioral health patients (as evaluated by the supervisor)Successful completion of nurse assistant training programCurrent BLS certification endorsed by the American Heart Association or the ability to obtain within 60 days of hire (with prior approval),Certified as a nurse aide in the State of ColoradoPreferred QualificationsCollege Degree (AS,BS)Physical Requirements : No Additional Description AvailableAnticipated job posting close date : 09 / 12 / 2024Location : LMC West Pines Behaviorial HealthWork City : Wheat RidgeWork State : ColoradoScheduled Weekly Hours : 36The hourly range for this position is listed below.
