STATUS : Full TimeLOCATION : Southern Monroe / Livingston CountyDEPARTMENT : Home CareSCHEDULE : Weekend Shifts - Work 30 hours and get paid for 40 hoursATTRIBUTESAssociate's Degree in Nursing required;

Bachelor's Degree in Nursing preferred.1 year of nursing experienced preferred. Prior home health, clinical and direct patient care experience preferred.

Ability to work independently.RESPONSIBILITIESPatient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home;

perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of carePlanning.

• Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed;

communicate with office and supervisory staff regularly; coordinates client services and / or referrals in an efficient and timely mannerDocumentation.

00 - $94,300.00The listed base pay range is a good faith representation of current potential base pay for successful applicants.
