JOB SUMMARY :

The Direct Support Para-Professional functions in the North Country Transition Home Program by providing assistance and support to residents with developmental disabilities and / or mental illness who are working toward the goal of transitioning to a less restrictive environment in their home communities.

The DSPP assists in the development and implementation of the individual plan of service, follows care plans, positive behavior support plans and assists residents in achieving their individual goals.

The DSPP assists residents in areas such as administering medications, personal care and community living supports. The DSPP participates with residents providing therapeutic experiences including DBT skills training, Motivational Interviewing, social skills groups, providing a culture of gentleness and skills training.

The DSPP is an integral part of the team and participates in all team meetings, staff meetings and in-services / training.

JOB REQUIREMENTS : Education : High School Diploma or equivalent

Experience : Prior experience in direct care human services. Computer skills.

Other : Satisfactory completion of training to meet requirements of regulating bodies and demonstrated ability to apply such training to the work environment.

Experiences with behavioral health issues are valued. ESSENTIAL FUNCTIONS :

  • The DSPP Supports residents in the achievement of goals and stabilization by assisting the case manager and Transition Home Administrator in the development and implementation of the individual plan of service.
  • Will be proficient in understanding and implementing each individual plan of service, care plan, and positive behavior support plan.
  • Practices Gentle Teaching concepts in all interactions with residents as well as develop rapport with residents and cultivate supportive relationships.
  • Ability to respond to complex resident needs and challenges within a culture of gentleness.
  • Will have a working knowledge in motivational interviewing, conflict resolution, DBT Skills training and mindfulness.
  • Assists and supports the resident in basic and advances self-care skills.
  • Under the supervision of the transition home administrator, team leader, or direct support professional, participates with the resident in therapeutic social, recreational, and task-oriented activities while maintaining appropriate therapeutic relationships.
  • Demonstrated ability to work effectively independently and as part of a team. Contributes to building a positive team spirit, exhibits objectivity and openness to others’ views, gives and welcomes feedback and support everyone’s effort to succeed.
  • Ability to maintain composure under pressure and demonstrates healthy interpersonal communication skills.
  • Under the supervision of the transition home administrator, team leader, or direct support professional, maintain clinical records as designated by policies and procedures such as, but not limited to;

documentation of assigned training classes, physical observation, significant observation notes, and any behavioral management programs.

  • Administers and records medication following the established policies and procedures.
  • Attends and participates in staff, team, and in-service trainings.
  • Provides First Aid and necessary emergency treatment as prescribed.
  • Perform all areas of job description without any physical restrictions, which would include lifting bending, and stretching.
  • Is alert and awake during entire scheduled working hours.
  • Reports to supervisor and / or the recipient rights officer any situation that poses a potential violation of abuse or neglect, including adult protective services violations.
  • Provides transportation for all community-based outings.
  • Be an advocate for all resident’s dreams, hopes and needs.
  • Promote safe living conditions for residents and staff.
  • Be a positive role model for residents, treats all people with dignity, demonstrates cultural competence and promotes a harassment-free environment.
  • Assist in meal preparation, clean-up, hygiene and daily maintenance of the home and agency vehicles.
  • Practice universal precautions, as exposure to blood-borne or air-borne pathogens may occur due to direct contact with others.
  • Know and follow emergency procedures for fires, tornadoes, accidents or serious incidents that may affect others.
  • Perform all duties as assigned by supervisor.

PHYSICAL REQUIREMENTS : Ability to walk, bend, stand, sit, lift to 50 pounds with or without assistance, stretch / reach, hear, see, hand / finger dexterity and drive a vehicle.

Position requires physical ability to complete and maintain certification in CPI techniques which will be a training class provided to the job incumbent.

  • Models the c1C / S vision.
  • Must be current in Adult CPR and First Aid
  • Must be current in Non-Violent Crisis Intervention (CPI)
  • Must be current in Gentle Teaching
  • Must pass all necessary training required by the State, any governmental oversight agency, or the employer, and any and all test must be successfully completed within the required time frames.
  • Be at least 18 years of age and able to transport consumers in agency vehicles
  • Three good character and work references
  • Have own phone with Multi-factor authentication (MFA) capabilities. Use of personal phone may be required to access Agency applications.
  • Have reliable transportation in carrying out agency duties
  • Possess a valid Michigan driver’s license and have a satisfactory driving record.
  • Ability to work within guidelines of the code of conduct, regulatory compliance plan and personnel policies
  • Supports a philosophy of service delivery that is recovery-based, person centered and culturally competent.
  • Must be able to complete required written reports and follow written and oral instructions related to the job
  • Must demonstrate the ability to respond to the cultural and socioeconomic differences present among the residents.
