Position Summary Ensure every patient receives a thorough, comprehensive, and individualized physical therapy regimen to ensure optimal patient outcomes.

Compensation : $70,000 - $80,000 Essential Duties and Responsibilities The essential functions include, but are not limited to the following : · Perform a thorough evaluation of incoming patients to curate and document a recommended treatment plan based on each patient’s individual needs, consulting with program director and / or referring providers as needed.

  • Provide exceptional, ongoing care to patients, assisting with all phases of physical therapy treatments and techniques.
  • Discuss treatments with patients, recording their conditions and responses to therapy in their medical record after each visit.
  • Complete daily patient care notes, progress notes, discharge notes, and other required forms as requested by referring providers, rehabilitation nurses, or insurance companies.
  • Collaborate with both scheduling and authorizations teams to help ensure fluid treatment progression. · Operate and apply knowledge of standard physical therapy equipment.
  • Maintain friendly, cordial relations with all patients, vendors, and colleagues. · Comply with state and Company standards of operations and · Adhere to the Company’s core values and H.

O.P.E. mission statement. · Regular participation and measurable involvement in marketing and / or business development efforts.

  • Additional duties as assigned. Required Qualifications · Master and / or Doctorate degree in Physical Therapy from an accredited program.
  • Appropriate PT licensure and Colorado state registration (in good standing). · Interest in pain management, including chronic, highly complex patients.

Minimum Qualifications (Knowledge, Skills, and Abilities) · 2-3 years with high medical complexity patient populations. · Certification in dry needling is preferred, but not required.

  • Knowledge of interventional pain management procedures is preferred, but not required. · Excellent oral, written, and listening communication skills.
  • Effective organization and time management skills with the ability to work in high-pressure situations. · Excellent interpersonal skills and demeanor.
  • Proficient in the use of a PC; in a Windows environment; in the use of the Internet; and in the use of MS Office Applications such as Outlook, Word, PowerPoint, and Excel.
  • Proficient in the use of EMR systems such as Modernizing Medicine ( EMA ). Ability to adhere to strict attendance and punctually expectations.

Ability to attend to details ensuring accuracy in a timely manner. Ability to work both independently, as well as collaboratively.

Ability to identify and proactively solve problems. Ability to thrive in a patient-focused environment.
