$36.02 - $54.04 an hour - Full Time, Days. 40 hours a week.
Boulder Community Health in Lafayette is seeking a Physical Therapist who assumes responsibility and accountability for providing quality patient care through the provisions of physical therapy services.

Qualifications
- Current Colorado state license for Physical Therapy.
- BLS certification required by the end of conditional period.
Responsibilities
- Selects and establishes physical therapy diagnosis.
- Formulates and maintains an individualized patient plan of care based on functional long and short term goals.
- Provides treatment using appropriate tools and demonstrates a complete knowledge of patient management.
- Evaluates patient response to treatment and revises plan of care as indicated.
- Promotes effective communication and teamwork with team members, the hospital and other contacts.
- Reports and documents evaluations, plans, treatment techniques, assessment, and patient progress.
- Initiates and is responsible for problem solving and decision making with relation to patient care.
- Functions as an advocate for the patient within the rehabilitation system.
- Participates in the achievement of department and hospital goals.
