We are currentlyseeking compassionate and dedicated Shared Living Providers forpersonalized support to individuals with unique preferences in ourcommunity! The ideal candidate will be able to create a comfortableand engaging environment tailored to the specific needs of eachindividual.

We have several people in the community seeking homeproviders, would you be interested in helping them find theirforever home?

Maleclient needs a quiet and relaxing living space with a designatedarea set-up with a TV for him to watch his favorite cartoons(Batman, Superman, Puppy Dog Pals).

Able to facilitate hisenjoyment of outdoor activities, such as sitting outside andobserving the surroundings. Assist him with mobility using hiswalker.

Male client seeks a living arrangementin a country or farm setting and incorporating elements related tohis interests such as John Deere and sharks.

Provide companionshipand engage in activities related to his preferred genres of Horrorand Westerns.

• Male client needs a wheel-chairaccessible environment. Facilitate his love for playing cards anddominoes. Facilitate his desire to attend church on Sundays andprovide opportunities for him to sit outside and listen tomusic.
• Female client needs a wheel-chairaccessible environment, as she uses a motorized chair. Support hercomputer and phone activities, as they are essential to her.

Sheenjoys watching TV in the evenings. Understand her commitment toher job at Vargas and the importance she places on it.

As a home provider, you share yourhome with an individual with a disability. Your responsibility isto maintain a safe and healthy home environment for the individualand be a mentor while helping the person as needed with the tasksof daily living.

In this role, you assist withcultivating life skills and fostering independence. We work tomatch interests and lifestyles between people .

You willparticipate together in activities of daily living , both at homeand in the community. Through the matching process, you are able tomatch with a person whose needs fit the support level you wish tooffer.

Duties vary for each individual from minimal support (i.e.,budgeting, medication, transportation, activities of daily living)to full support (i.

e., dressing, toileting, feeding, positivebehavioral support, etc.) , and the income is commensurate with thelevel of support provided.

Responsibilities include :

• Provide support for tasks such as cooking, cleaning,laundry, budgeting, decision-making, etc.
• Skill acquisition
• Medicationadministration
• Medical appointments
• Support with overall health and well-being
• Daily documentation via web-based case managementapplication (THERAP)
• Provide fun and leisureexperiences to the individual by providing opportunities ofcommunity inclusion, integration and socialization
• Compliance with all state / agency policy andprocedures
• Communicate effectively with teammembers as needed
• Support physical needs ofindividual served
• Providetransportation

Possibleresponsibilities depending on your preference :

• Assistance with bathing, toileting, feeding,behavioral support, etc.
• Provide support formedical protocols, such as seizure, fall, feeding tube,etc.
• Care for someone with complexphysical / medical needs (i.e., hearing impaired, vision impaired,non-ambulatory, etc.)
• Potential support ofbehavioral issues.
• Must be a resident of the state in which you wish to providecontracted services in
• Transportation, validdriver's license, car insurance and current registration
• Active home / renters insurance
• Own or rent your own home / apartment / condo / townhouse
• Must be at least 18 years of age
• Experience preferred but not required
• Reliable computer and internet access
• Complete all state mandated trainings
• Environmentally safe and clean residence
• Ability to pass State / Federal and APS / CPS backgroundcheck
