Summary :
The Licensed Practical Nurse (LPN) will support the LMCMS team and individuals in services in the oversight and delivery of medical activities.
Essential Duties and Functions :
1. Work collaboratively with the Medical Assistants (MAs) to provide comprehensive physical health care for all individuals.
2. Manage patient flow in Outpatient setting in collaboration with the team.
3. Provide physical health services to patients of all ages, including phone / walk-in triage.
4. Provide education, counseling, and family planning activities and coordinate appointments and referrals and assist in obtaining needed services and supplies.
5. Manage prior authorization of prescriptions and treatments as well as assist with problems concerning medication refills that are not within the routine refill process established at LMCMS.
6. Maintain laboratory standards and policies for designated level at facility.
7. Assigns, performs, and assesses vital sign checks conducted on individuals in service.
8. Oversees the recording of necessary medical documentation into the Electronic Health Record (EHR) and maintains proper documentation in the EHR of all significant interactions with or on behalf of individuals in service.
9. Maintains the assignment of MAs to other responsibilities that maintains a neat and orderly environment that meets OSHA standards.
10. Assures that yearly competencies are done, as necessary.
11. Participate in health education and promotion activities in collaboration with the Unit Director, Medical Director, Clinical Director and Integrated Health Manager.
12. Maintain HIPAA regulations via training.
13. Maintenance of a trauma informed environment that supports confidentiality.
14. Attendance and participation in meetings, in-services, educational programs and other activities as deemed appropriate.
15. Implement and maintain quality improvement activities related to Primary Care, in collaboration with the Unit Director, Director of Operations, Quality Assurance Manager and the Clinical Director.
16. Maintain a current LPN license and required number of continuing education credits.
17. Supervision of the inventory of all medications, medical supplies, Family planning supplies, sample medications, education materials;
contact / order appropriate materials when needed in collaboration with the Director and Office Manager.
18. Assist in and coordinate the collection of urine samples for Urine Drug Screenings.
19. Assist Integrated Health Manager with training new MAs in medical activities by verbal instruction and shadowing.
20. Other duties and responsibilities as assigned.
Qualifications :
High school diploma or GED required; Bachelor's Degree preferred
At least 2 years' experience as an LPN; experience in an outpatient practice setting preferred
Current LPN license from the Commonwealth of Pennsylvania
This position is based in a community mental health clinic.
