Physical Therapist Home Health

VNA of Ohio is now hiring a Full-Time Physical Therapist.
The Physical Therapist (PT, DPT) in Home Health will be responsible for the assessment and evaluation of patient care needs related to functional status / mobility, endurance / stamina, pain and other medical conditions.
Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

Responsibilities
Essential Functions :
- Performs initial assessment or admission to agency and determines patient treatment needs
- Develops a plan of care with measurable goals and specific discipline related treatments in conjunction with physical referral / orders
- Provides direct physical therapy services as outlined in the plan of care including performance of physical therapy procedures, instruction of patient and / or caregiver, recommendations for equipment or adaptation of home environment
- Re-evaluates and revises plan of care as needed with physician approval
- Facilitates total care services to patients by making appropriate referrals to other disciplines by obtaining physicians' orders or by consulting with case manager
- Submits all documents in a timely manner
- Performs other related duties as assigned
Qualifications
Qualifications and Requirements :
- Bachelor of Science Degree in Physical Therapy required, post-graduate degree preferred
- One year experience in a supervised healthcare setting
- Licensed, in good standing, to practice physical therapy in the State of Ohio
- CPR certification required
- Valid Driver's License
